2. Read the eligibility criteria and instructions carefully
3. Follow the link to the Ontario Honours and Awards Portal
4. Create an account (or sign in for returning users) on the Ontario Honours and Awards Portal
5. Submit your nomination.
    

The deadline for application is May 31, 2025.
