ODA is collecting information from dental providers outside of the tri-counties who still need access to the COVID-19 vaccine. Please complete this form if you still need access to the COVID-19 vaccine. We will share the information with the Oregon Health Authority (OHA), who will coordinate with local public health departments to get dental professionals scheduled for COVID-19 vaccinations.

The two mass vaccination sites offering vaccine appointments in the metro area are the All4Oregon Mass Vaccination Site at the Oregon Convention Center and the PDX Airport drive-through site. Seniors with mobility needs, and individuals with disabilities and their caregivers will continue to be offered appointments at the PDX Airport drive-through site. All other groups seeking appointments at the Oregon Convention center should sign up at OHA's Get Vaccinated Tool to be notified when future appointments are available.
