Through its representation, advocacy and education functions, the Case Management Society of America (CMSA) has been an organizing force setting the practice direction for the discipline of case management. CMSA promotes practice that is evidence-based and discourages the use of practices which, though popular or widely accepted, are either not beneficial or are contrary to the CMSA Standards of Practice for Case Management (SoP). While the increasing emphasis on care coordination by providers and payers has opened professional debate regarding the models being used in hospitals today, CMSA intends to clarify its position on hospital case management practice through this white paper initiative and urges hospital leaders to seek solutions that more effectively address the needs of our most vulnerable hospitalized patients.

CMSA's Standards of Practice for Case Management, 2016 revision, provides practice guidelines for the case management industry and its diverse stakeholders. The impetus for the 2016 revision of the Standards is the need to emphasize the professional nature of the practice and role of the case manager. The 2016 Standards of Practice contain information about case management including an updated definition, practice settings, roles and responsibilities, case management process, philosophy and guiding principles, as well as the standards and how they are demonstrated.

The Holiday Season, including the celebrations of Christmas, Hanukkah, Kwanzaa, Diwali, Las Posadas, Winter Solstice and the Chinese New Year is a happy, joyous time for most of us, as we look forward to spending time with our extended family and friends. However, for many people who live alone, have few friends, feel lonely or socially isolated, these days are no different than every other day for them. The feelings of loneliness and isolation are even more accentuated at this time of year.

 

This made me think about what we really know about loneliness. We know it causes problems for many people, but what has caused the increase in loneliness that is being seen across all age groups? And how many clinicians are aware of the significant health risks associated with loneliness, especially in elderly patients?  Are people aware it is being called the “Silent Killer of the Elderly”?  What is being done to educate people about the risks of loneliness going untreated?  How many people suffer from loneliness in the U.S. and is there a “loneliness epidemic” in the U.S.?  And what should we call it?  Is it a mental state? A concept? A symptom? A disorder? A condition? Or a disease?

For the third year in a row, the past presidents of the Case Management Society of America participated in a general session during the annual conference. The session for 2019 was titled “Legacy and Leadership: CMSA 2019 Presidents’ Panel.” The concept of bringing the past presidents together is to tap into the individual expertise each of the past leadership of CMSA had to share with the attendees of the conference. Most of the past presidents are in active practice and use the expertise they have gained as industry leaders to advance the practice of case management across the broad healthcare continuum.

 

Many of the past presidents sit on boards, are consultants to hospital and managed care upper management and take the time to mentor those in practice as well as those new to the practice. In other words, all of the past presidents of CMSA are ambassadors to the Case Management Society of America and professionals who make up the practice across the care continuum. This year’s panel included the following participants:

Total national healthcare spending in 2018 grew 4.6 percent, which was slower than the 5.4 percent overall economic growth as measured by Gross Domestic Product (GDP), according to a study conducted by the Office of the Actuary at the Centers for Medicare & Medicaid Services (CMS) and published today ahead of print by Health Affairs.

 

As a result, the share of the economy devoted to health spending decreased from 17.9 percent in 2017 to 17.7 percent in 2018.  Growth in overall healthcare spending has averaged 4.5 percent for 2016-2018, slower than the 5.5 percent average growth for 2014-2015, that was affected by expanded Medicaid and private insurance coverage and increased spending for prescription drugs, particularly for drugs used to treat hepatitis C. The growth in total national healthcare expenditures was approximately 0.4 percentage point higher than the rate in 2017 and reached $3.6 trillion in 2018, or $11,172 per person.
