CMSA's Standards of Practice for Case Management, 2016 revision, provides practice guidelines for the case management industry and its diverse stakeholders. The impetus for the 2016 revision of the Standards is the need to emphasize the professional nature of the practice and role of the case manager.

The 2016 Standards of Practice contain information about case management including an updated definition, practice settings, roles and responsibilities, case management process, philosophy and guiding principles, as well as the standards and how they are demonstrated.

The case management profession has changed dramatically in the past 25 years. In the late 1990s, case managers primarily worked in acute and post-acute facilities, health plans, medical management organizations and workers’ compensation companies.That began to change in the early part of the 21stcentury with major changes that occurred in the healthcare industry. More emphasis was being placed on finding ways to deliver more efficient, quality care that was also more cost-effective. Healthcare was moving from a fee-for-service payment system that was based on the quantity and volume of services to a value-based system that was based on the quality and value provided by those services. New care delivery models and healthcare reforms, brought about by the Patient Protection and Affordable Care Act, meant that new innovative ways of delivering care needed to be developed. This certainly helped to spotlight case managers and the value they could bring to these programs.

The relationship between a patient and provider isn’t singular; rather, it is a relationship between the patient and an entire practice. Without the patient engagement team, which is quite often led by a primary care case manager (PCCM), a patient can fall through the cracks.

As an example, a 78-year-old female patient schedules an appointment with her primary care physician to get her diabetic medications refilled but doesn’t show for the appointment. The patient runs out of medications and ends up in the ED with hyperglycemia, where she receives a new prescription and is discharged home — an expensive and unnecessary step that the patient care team, led by a case manager, should prevent.

Proposed changes to the Medicare Physician Fee Schedule and Quality Payment Program would streamline clinician billing and expand access to high-quality care

On July 12, the Centers for Medicare & Medicaid Services (CMS) proposed historic changes that would increase the amount of time that doctors and other clinicians can spend with their patients by reducing the burden of paperwork that clinicians face when billing Medicare. The proposed rules would fundamentally improve the nation’s healthcare system and help restore the doctor-patient relationship by empowering clinicians to use their electronic health records (EHRs) to document clinically meaningful information, instead of information that is only for billing purposes.

"Today’s reforms proposed by CMS bring us one step closer to a modern healthcare system that delivers better care for Americans at a lower cost," said HHS Secretary Alex Azar. "Such a system requires empowering American patients by giving them price and quality transparency and control over their own interoperable health records, goals supported by CMS’s proposals. These proposals will also advance the successful Medicare Advantage program and accomplish a historic regulatory rollback to help physicians put patients over paperwork. Further, today’s proposed reforms to how CMS pays for medicine demonstrate the commitment of HHS to implementing President Trump’s blueprint for lowering drug prices. The ambitious reforms proposed by CMS under Administrator Verma will help deliver on two HHS priorities: creating a value-based healthcare system for the 21st century and making prescription drugs more affordable."

Significant advancements in multiple sclerosis care, including the arrival of new therapies, have made treatment decision-making and navigating health insurance requirements increasingly complex for patients and their providers.To simplify the process, PRIME worked in partnership with the CMSA and the Academy of Managed Care Pharmacy (AMCP) Foundation to develop myMS.care, a new online tool featuring 2 complimentary web apps. The web apps – a CarePath for patients, and a CareNavigator for providers – are tethered to facilitate shared decision-making and help guide critical conversations between those receiving care and those delivering it. Aligning preferences and goals to ensure patients and providers are on the same page often leads to greater satisfaction and improves the patient experience throughout the course of the MS journey.By offering specific tips on efficiently navigating the insurance process, the web apps are instrumental in avoiding unnecessary delays in treatment.Additionally, patients using the web app are able to print a customized "care guide" to take with them to their medical appointments. The care guide, which includes a list of the patient’s preferences, key concerns, and questions, serves as an important resource and reference tool when patients are deciding on a care plan with their providers.
