CMSA's Standards of Practice for Case Management, 2016 revision, provides practice guidelines for the case management industry and its diverse stakeholders. The impetus for the 2016 revision of the Standards is the need to emphasize the professional nature of the practice and role of the case manager.

The 2016 Standards of Practice contain information about case management including an updated definition, practice settings, roles and responsibilities, case management process, philosophy and guiding principles, as well as the standards and how they are demonstrated.

For many patients with idiopathic pulmonary fibrosis (IPF) and their caregivers, the journey to diagnosis and appropriate management is often riddled with gaps in care coordination and interprofessional collaborative practice. With the goal of improving holistic transitions of care to minimize hospitalizations and readmissions, and to ensure incorporation of evidence-based treatment and management approaches, this webcast brings to light core concepts of the IPF Transitions of Care (TOC) Pathway that was recently developed in partnership with the National Transitions of Care Coalition (NTOCC) and Case Management Society of America (CMSA). Led by key members of the IPF TOC Pathway Steering Committee, this webcast was designed to arm system leaders and the IPF care team with essential interventions and tools to ensure effective and safe transitions of care for patients with IPF across healthcare settings.

The Association of American Medical Colleges predicts that by 2020 the U.S. will be facing a shortage of 90,000 physicians, including 45,000 patient care physicians. This shortage is a result of the large number "baby boomer physicians" who will be retiring this decade. In 2010, 26 percent of active physicians were over 60 years of age.

At the same time, the baby boomer generation and longer life spans have created an unprecedented growth in the number and proportion of older adults. The State of Aging & Health in America in 2013 report by the Centers for Disease Control and Prevention predicts the population of Americans aged 65 years or older will increase to about 71 million by 2030, and those 80 or older will increase to 19.5 million. In 2030, when the last baby boomer turns 65, older adults are expected to account for 19.6 percent of the U.S. population. These numbers will be double what they were in 2005.

The POLST refers to Physician Orders for Life Sustaining Treatment, and it is truly recognized as a full-fledged program, not just another life care planning document. The program was initiated by a group of Oregon physicians who were continually seeing similar circumstances with their end-of-life patients. Those circumstances included patients without clearly documented treatment choices, incomplete clinical information and insufficient or lack of advance directives (AD). After two years of collaboration to discuss the gaps in advance life care planning activities, Oregon initiated the POLST program in 1991. Currently, all but five states in the U.S. have a POLST program in place, although the maturity of the program varies greatly based on when it was initiated. The POLST form is viewed in the medical community as not merely a checklist, but rather a tool to guide meaningful conversation regarding patient wishes and treatment ramifications. Other key features of the document itself include that it is easily identifiable, succinct, portable across all care settings and delineates specific treatment choices. The form is divided into separate sections, which include CPR Initiation, Medical Interventions, Organ Donations, Patient Representative Agents, and patient and physician signatures.

New helpline launched to provide additional support

On February 17, the Centers for Medicare & Medicaid Services (CMS) awarded approximately $20 million to 11 organizations for the first year of a five-year program to provide on-the-ground training and education about the Quality Payment Program for clinicians in individual or small group practices of 15 clinicians or fewer. CMS intends to invest up to an additional $80 million over the remaining four years.

These local, experienced, community-based organizations will provide hands-on training to help thousands of small practices, especially those that practice in historically under-resourced areas including rural areas, health professional shortage areas, and medically underserved areas. The training and education resources will be available immediately, nationwide, and will be provided at no cost to eligible clinicians and practices.
