Care managers are an integral part of managing a patient’s health—ensuring patients adhere to their care plans, providing advice and information when patients have questions, and helping patients become and remain healthy. However, the positive outcomes associated with proper care management cannot be achieved by the care manager alone. They need a total commitment from the patient to become involved, since patient engagement is a critical element of successful care management.

A new report released on Sept. 12 by the Department of Health and Human Services (HHS) shows that 6.8 million consumers saved an estimated $1.2 billion on health insurance premiums in 2012, due to the "rate review" provision of the Affordable Care Act, which brought unprecedented accountability to slow the growth of health insurance premiums. The Affordable Care Act, along with state efforts, continues to bring scrutiny to proposed health insurance rate increases and is saving consumers real money as a result.
 
CMSA has launched CMAG 2012, the third revision of the resource, which is designed to provide case management professionals with assessment and intervention tools to improve patient medication and treatment adherence.

The information and tools presented in this edition serve the professional care management community as the standard by which case managers can advance assistance, coaching and monitoring efforts with the goal of ultimately improving care delivery and patient outcomes.
