The Case Management Society of America Foundation (CMSAF) is offering a scholarship for the Spring 2019 Integrated Case Management Training Program, presented by the Case Management Society of America. The deadline to submit has been extended to March 25, so there's still time for your application!
 
CMSAF is offering this scholarship to a case manager who is committed to improving the health outcomes of the most at-risk populations and is willing to link and integrate care coordination across all domains of health. At-risk populations include individuals who have multiple chronic conditions, the frail, elderly, children with special healthcare needs, those with co-morbid medical and behavioral conditions, and may include the presence of social or health system barriers. Person-centered Thinking is the foundation of Home and Community-based Services. If you are a case manager working in long-term care or in home and community-based care, this workshop is for you! A certificate of completion is awarded at completion. Person-centered thinking recognizes that people are experts in their own lives; everyone can express their preferences and live a full life in their own community that they and the people who care about them have good reasons to value.
 
• Person-centered planning identifies and addresses the preferences and interests that make up a desired life and the supports (paid and unpaid) needed to achieve it. It is directed by the person and supported by others selected by the person, who are independent of any service/support to be delivered in the plan.
 
• Person-centered practice is the alignment of service resources and systems that give people access to the full benefits of community living and delivers services in a way that facilitates achieving the person’s desired outcomes.
 
The final CMS rules for Home and Community-based Services, 42 CFR 438.208(c)(3) and 41.301(b)&(c) state: "A consumer's service plan must be developed by individuals who are trained in Person Centered Planning.”
 
However, before you develop a person-centered plan, you must learn to think in a person-centered manner to better understand their needs and preferences. Attend the workshop at CMS Annual this summer in Las Vegas; learn more and register at the link!

Technology today makes the healthcare industry entirely different from what it was when I started my nursing career. There were no computers to track and monitor the numerous processes in the hospital; no cell phones; no automatic devices for IV drips, respiratory machines, or telemetry; no automatic thermometers or BP devices; no electronic medical records; and no electronic pharmacy ordering/delivery system. Everything was done manually. Surgery schedules, staff schedules, admitting logs, and all other informational documents were typed or handwritten anew each day. Consequently, it took a long time to get anything accomplished, yet we were able to provide excellent care considering what we had to work with.
 
Compare that to today, when we are used to having everything electronic and at our fingertips.  Processes in the hospital are electronic and automated — computerized records, all types of electronic monitoring devices, order entry for all ancillary services, remote monitoring, computer programs for all aspects of patient care, cell phones for instant communication with others and access to internet information, and computerized logs, reports, and any other type of data that is available instantly in real time. Consequently, things get accomplished very quickly today, and most of these things are even done automatically without us having to intervene.

In this issue, themed around integrated case management, we discuss the important work being done through evidence-based practice (EBP). When we practice based on evidence, we integrate the individual case manager’s clinical expertise with the best available external clinical resources, while also taking into account the individual patient’s own values. This implementation into all aspects of the shared decision-making processes yield a full, comprehensive assessment and a subsequent professional case management service of care toward the goal of positive outcomes.
 
In the Professional Case Management journal, Suzanne Powell writes: “Evidence-based guidelines are becoming essential tools in the practice of case management. They serve as that valued resource that allows the individual case manager to be proactive in planning, educating the patient and the multidisciplinary team” (Powell and Commander, 2007). Evidence-based care management does not rely on opinions, but rather on actual evidence examination. Evidence enhances our practice by providing additional tools needed for the case manager to ensure the delivery of safe, quality care.
 
Supporting ambulance triage options aims to allow beneficiaries to receive care at the right time and place 
 
On March 11, the Centers for Medicare & Medicaid Services (CMS) is announcing a free on-line training course in infection prevention and control for nursing home staff in the long-term care setting developed with the Centers for Disease Control and Prevention (CDC).
 
The course information covers core activities for an infection prevention and control program, with a detailed explanation of recommended practices to prevent pathogen transmission and reduce healthcare-associated infections and antibiotic resistance in nursing homes.

Building on the success of ibd.care and myms.care, PRIME, in partnership with the Case Management Society of America (CMSA) and the Academy of Managed Care Pharmacy (AMCP), are pleased to announce the launch of lipid.care, a new online-tethered tool to facilitate shared decision making and help guide critical conversations between patients receiving care for high cholesterol and the clinicians delivering it. Accessed from the same landing page and requiring no download, patient-focused CarePath and provider-focused CareNavigator help align preferences and goals, ensuring that patients and their providers are on the same page to help improve the patient experience and outcomes. These online tools are also designed to help simplify the process of navigating increasingly complex health insurance requirements for both patients and providers by offering specific tips to avoid unnecessary delays in treatment. In addition, patients can print a customized “CareGuide” to take with them to their medical appointments. The CareGuide, which includes a list of the patient’s preferences, key concerns, and questions, serves as an important resource and reference tool when patients are deciding on a care plan with their providers.
