Launched in 2014, CMSA’s Career & Knowledge Pathways (CKP) educational program was developed to fill a longstanding gap in case management education. 

The debut CKP course, CMSA Standards of Practice: The Foundation for Professional Excellence in Coordination of Care Across the Continuum, was developed by thought leaders and your fellow CMSA colleagues to contribute to the basic understanding of the standards that govern our practice.

Based on CMSA’s Standards of Practice for Case Management, this tool is a contextual learning program that covers each of the standards and gives you the opportunity to learn at your own pace. Where traditional educational programs are limited in content, structure, mode of delivery, or timing, CKP uses multiple delivery methods including video, simulations, and case studies to encourage a collaborative and flexible learning experience.

CKP is made up of 16 modules, providing 60 hours of continuing education for nurses, social workers, and case management certification.

 

CMSA is pleased to offer an updated version of the CMAG Diabetes Disease State workbook. Click here to download this workbook.

The Case Management Adherence Guide (CMAG) provides case managers of any background with the information and tools necessary to advance assistance, coaching, and monitoring efforts with the goal of ultimately improving patient outcomes and care delivery.

Disease-specific chapters provide case managers with more in-depth information on how to apply the CMAG principles to specific disease states and patient populations.

The Centers for Medicare & Medicaid Services (CMS) for the first time introduced star ratings on Hospital Compare, the agency’s public information website, to make it easier for consumers to choose a hospital and understand the quality of care they deliver. This announcement builds on a larger effort across HHS to build a health care system that delivers better care, spends health care dollars more wisely, and results in healthier people.

Medicare is celebrating its 50th Anniversary! Do you know someone who has benefited from the Medicare Program? We have a great opportunity for them to share their stories and testimonials! Attached are two forms:

1. Share Your Medicare Experience - Where we ask for basic information from the beneficiary as well as their story/testimonial.

2. Release Form - To give us permission to share their story/testimonial on our websites or via social media.

AHRQ releases new continuing-education video on medication reconciliation supported by clinical pharmacists

A new continuing-education video from the Agency for Healthcare Research and Quality's (AHRQ) Effective Health Care (EHC) Program will explore how medication reconciliation can help prevent medication errors during transitions in care. Some evidence shows that pharmacist-led processes could prevent medication discrepancies and potential adverse drug events after discharge. The video is available at http://bit.ly/1DAQjgC.
