Medicare's 50th Anniversary-Testimonials and Stories

Medicare is celebrating its 50^th Anniversary! Do you know someone who has benefited from the Medicare Program? We have a great opportunity for them to share their stories and testimonials! Attached are two forms:

1. Share Your Medicare Experience - Where we ask for basic information from the beneficiary as well as their story/testimonial.

2. Release Form - To give us permission to share their story/testimonial on our websites or via social media.

Please share this information with your patrons and ask them to send their stories. Ask them to fill out the attached form and the information release, and send it to crystal.barnes@cms.hhs.gov.
