Based on the Integrated Case Management Manual (2nd Edition), the training reflects decades of research showing that coordinated, holistic care improves outcomes and strengthens care transitions.

The ICM model recognizes that physical health, behavioral health, and social determinants are deeply interconnected. It provides a structured framework to assess these factors, engage clients more effectively, and develop coordinated care plans that lead to improved well-being.

Participants learn practical strategies for risk stratification, communication, care planning, and outcome measurement through a blend of self-study and live, interactive sessions. Case studies, role-play, and discussion help translate concepts into real-world application.

The impact is significant. Case managers are better equipped to stabilize high-risk patients, improve interdisciplinary collaboration, and demonstrate meaningful outcomes. Participants earn 28.5 RN, SW, and CCM CEs while gaining skills they can immediately apply in practice.

As healthcare shifts toward value-based care and population health, the ability to manage complex cases is more critical than ever. This training helps both individuals and organizations strengthen outcomes and deliver truly person-centered care.

“Improving Hypertension Control in the Black and African American Population”

Hypertension remains a leading risk factor for cardiovascular and renal disease, with persistent disparities in prevalence and control. An analysis by our Population Health team found that Black and African American patients were 7% less likely to have controlled hypertension than other groups. This is especially concerning given they are 30% more likely to develop hypertension, highlighting a significant and ongoing health inequity.
