As case managers, you know that innovation in healthcare is constant. One of the most significant changes we’ve seen since the pandemic is the rise of “Hospital at Home” programs. These programs became a critical tool to help hospitals manage capacity, but with the CMS waiver that made them possible set to expire, what's next?
This informative webinar will provide you with the information you need to understand the future of this care model. Our expert presenters will walk you through:
- The status of the pending legislation that could allow these programs to continue.
- A data-driven look at outcomes: How do readmission and mortality rates for hospital-at-home patients compare to those in a traditional hospital?
- The bottom line: We'll break down the costs and reimbursement models for these programs.
This is a fantastic opportunity to stay ahead of the curve and understand how these changes could impact your practice and the patients you serve.
Meet the Speakers:
- Amy S. Chartier, BA, MSN, RN, CMGT-BC
- Margaret “Meg” Flynn, RN, BSN, CCM
Register today and be part of the conversation!

Hospital at home is not a new medium of acute care; however, during the pandemic, it became more mainstream to help alleviate capacity struggles. In 2020, the Centers for Medicare & Medicaid Services (CMS) initiated a waiver called Acute Hospital Care at Home (AHCAH) under section 1135. This waiver allowed hospital systems to offer acute inpatient care to patients in their own homes with greater flexibility. This waiver is set to expire soon, and many healthcare organizations are hesitant to continue their hospital-at-home programs without its support.
