In order to maintain the necessary care coordination and transitions of care for the injured worker, communication between the acute care case managers and the worker’s compensation case managers is critical.
Case managers are routinely confronted by gray areas, which occur when there are multiple stakeholders demanding critical thinking. Regardless of title or profession of origin, practitioners must appreciate that dealing with ethical dilemmas acknowledges there may not be a clear right and wrong when a conflict occurs. Valuable resources such as the CCMC professional code of ethics, awareness of the 5 basic ethical principles, NASW Code of Ethics and CMSA Standards of Practice, should be viewed as practice guidelines and provide an excellent foundation in all disciplines. They improve the quality of care in the emerging models of care, and transitions of care in practice settings, highlighting the importance of communication among all providers and practitioners.

While the increasing emphasis on care coordination by providers and payers has opened professional debate regarding the models being used in hospitals today, CMSA intends to clarify its position on hospital case management practice through this white paper initiative and urges hospital leaders to seek solutions that more effectively address the needs of our most vulnerable hospitalized patients.

Download CMSA's Standards of Practice for Case Management, 2016 revision, which provides practice guidelines for the case management industry and its diverse stakeholders. The impetus for the 2016 revision of the Standards is the need to emphasize the professional nature of the practice and role of the case manager.

The 2016 Standards contain information about case management including an updated definition, practice settings, roles and responsibilities, case management process, philosophy and guiding principles, as well as the standards and how they are demonstrated.

At the Past Presidents’ Panel this June in Las Vegas, all the participants were asked specific questions, based upon our own expertise and experience. With our multiple years ‌of hands-on leadership, we realized that many of the attendees might like to hear our perspective in building successful case management departments, no matter the venue.
My specific expertise has been in multiple settings, but for this publication, I wanted to share some insight into acute care case management departments.
I have consulted in multiple acute care settings across the nation. There are several things that I feel lead to the success of the department. One critical aspect is to whom in the C-suite the department reports directly. Without the support of this level of administration, the department could be set to fail. I have reported through the nursing officer, the chief financial officer and the chief medical officer at any one time. My preference is to report through the CFO directly, with strong dotted lines to the CNO and the CMO.

The Centers for Medicare & Medicaid Services (CMS) on September 26 issued a final rule that empowers patients to make informed decisions about their care as they are discharged from acute care into post-acute care (PAC), a process called “discharge planning.” In addition to improving quality by improving these care transitions, the rule supports CMS’ interoperability efforts by promoting the seamless exchange of patient information between health care settings, and ensuring that a patient’s health care information follows them after discharge from a hospital or PAC provider.
