Building on the success of ibd.care and myms.care, PRIME, in partnership with the Case Management Society of America (CMSA) and the Academy of Managed Care Pharmacy (AMCP), are pleased to announce the launch of lipid.care, a new online-tethered tool to facilitate shared decision making and help guide critical conversations between patients receiving care for high cholesterol and the clinicians delivering it. Accessed from the same landing page and requiring no download, patient-focused CarePath and provider-focused CareNavigator help align preferences and goals, ensuring that patients and their providers are on the same page to help improve the patient experience and outcomes. These online tools are also designed to help simplify the process of navigating increasingly complex health insurance requirements for both patients and providers by offering specific tips to avoid unnecessary delays in treatment. In addition, patients can print a customized “CareGuide” to take with them to their medical appointments. The CareGuide, which includes a list of the patient’s preferences, key concerns, and questions, serves as an important resource and reference tool when patients are deciding on a care plan with their providers.

It’s that time of year again – time to look at the healthcare trends and predictions for 2019. It’s always interesting to look at what the healthcare experts think will be the “hot topics” for the coming year. Since there are numerous areas in healthcare that publish predictions, I chose to focus on the health information technology (HIT) and medical innovations areas. The following lists were created by reviewing numerous websites, studies, surveys, and articles.

Health Information Technology Predictions
• Artificial Intelligence (AI) and Machine Learning
• Innovative New Drugs, Precision Medicine, Therapies, Devices and Wearables
• Blockchain
• Big Data and Data Ownership
• Digital Health and Patient Experience
• Healthcare Insurance Changes
• Value-Based Care
• Cybersecurity

Medical Innovations
• Stroke Screening and Intervention
• Robotic Surgery
• 3D Printing
• Virtual and Mixed Reality

I can’t say many of these were surprises, but there were a couple that I did not think would make the list and others that I was surprised to see not on the list.

Quality improvement (QI) has been woven into the fabric of case management practice since the first CMSA Standards of Practice for Case Management were released in 1995. Since then, and through three revisions to the Standards of Practice, the consistent delivery of improved care quality has remained a core element of case management practice. With recent developments in U.S. health policies and value-based reimbursement models that mandate conducting QI initiatives, some healthcare teams and entire systems are just catching up with case management’s ongoing focus on quality. How can we best utilize our QI experiences in case management, drawing on our collaborative practice skills, to help healthcare teams effectively implement leading QI principles and methods to achieve national goals of quality-driven and value-based healthcare? In this article, I’d like to share a few suggestions and insights based on the many results of research-informed QI projects that my organization (PRIME Education) has published in the literature.

New Medicare cards offer better identity protection for millions of Americans

The Centers for Medicare & Medicaid Services (CMS) recently completed a large-scale effort to provide new Medicare cards without Social Security numbers to people with Medicare. The new cards support the agency’s work to protect personal identity and reduce fraud and abuse. Over the past nine months, CMS sent new cards to more than 61 million people with Medicare across all U.S. states and territories, completing the mailing ahead of schedule.
