Each year, CMSA pays special tribute to those individuals who have demonstrated distinguished service to the case management profession. A primary goal of the awards program is to inform members of the case management profession, other health professionals, government officials, and the public of the outstanding accomplishments and significant achievements of individual case managers in the delivery of health care. In addition, the awards program assists the public in gaining a better understanding of the role of case management and its significant contribution to the delivery of health care. CMSA and the board of directors are pleased to offer CMSA's Standards of Practice for Case Management, 2016 revision, which provides practice guidelines for the case management industry and its diverse stakeholders. The impetus for the 2016’s revision of the Standards is the need to emphasize the professional nature of the practice and role of the case manager.

The 2016 Standards contain information about case management including an updated definition, practice settings, roles and responsibilities, case management process, philosophy and guiding principles, as well as the standards and how they are demonstrated.

But now that it’s finished, I’m sure half of you are happy and looking forward to new beginnings and the other half are disappointed and worrying about what is to come. Working in healthcare, the big issue for me is what kind of healthcare changes are coming? I thought I knew what to expect if Secretary Clinton won -- maybe some needed revisions to the ACA, but it would have remained. However, I’m not sure what it will be like under President-elect Trump. Will the Affordable Care Act (ACA), also known as Obamacare, really be repealed? Will over 21 million people lose their healthcare benefits or will the ACA just be replaced with other alternative options? If so, what might those be? What will happen to those who have pre-existing conditions? Will children up to the age of 26 continue to be included in their parents’ plan? Will other principles, policies, and practices established over the past 6 years to support the ACA (Triple Aim, the move from fee-for-service to value-based care, ACOs, etc.) also be eliminated? Will health information technology (HIT) be affected? Will Health Information Exchanges be eliminated? Millions of dollars have been spent building an infrastructure to support the ACA. Will all that be useless now?

by Mary McLaughlin-Davis, DNP, RN, MSN, ACNS-BC, CCM The case manager establishes a relationship with his or her patient as the first step in assisting that patient to experience optimal health. It is in the initial patient assessment that the case manager determines if the patient is able to speak English, and if not, accommodations are made to communicate through an acceptable interpreter. Language is one potential barrier in the relationship between the case manager and the patient. However, there is another, less obvious roadblock to communication and relationship in serving diverse patient communities. Case managers must be cognizant of how our patients’ health literacy impacts their decision-making capability and health-related outcomes. The awareness of the importance of a patient’s health literacy directs the case manager’s plan for patient education and care management. The Institute of Medicine (IOM), The Agency for Healthcare Research and Quality (AHRQ) and the American Medical Association (AMA) have all issued reports on health literacy. All reported that as many as one-half of all adults lack the literary skills needed to function and navigate in the healthcare system. Patients with limited health literacy come from all segments of society, and most are white, native-born Americans (Weiss, et al., 2005).
Medicare finalizes policies to expand the Diabetes Prevention Program Model On November 2, the Centers for Medicare & Medicaid Services (CMS) finalized the 2017 Physician Fee Schedule final rule that recognizes the importance of primary care by improving payment for chronic care management and behavioral health. The rule also finalizes many of the policies to expand the Diabetes Prevention Program model test to eligible Medicare beneficiaries, the Medicare Diabetes Prevention Program (MDPP) expanded model, starting January 1, 2018. This is the first time a prevention model from the CMS Innovation Center will be adopted under the CMS authority to expand successful payment and service delivery models to reach all eligible beneficiaries. The 2017 Physician Fee Schedule final rule is one of several rules that reflect a broader Administration-wide strategy to create a health care system that results in better care, smarter spending, and healthier people. Proposals designed to help consumers share their personal health data easily and securely The Department of Health and Humans Services’ Office of the National Coordinator for Health Information Technology (ONC) on November 2 announced the Phase 1 winners of the Move Health Data Forward Challenge. Winners were selected based on their proposals for using application programming interfaces (API) to enable consumers to share their personal health information safely and securely with their health care providers, family members or other caregivers. "As health information technology becomes more accessible, consumers are playing an even greater role in how and when their health information is exchanged or shared," said Dr. Vindell Washington, national coordinator for health information technology. "The Move Health Data Forward Challenge will help consumers unleash their health data and put it to work." Source: U.S. Department of Health and Human Services
