Pulmonary arterial hypertension (PAH) has a considerable impact with high rates of morbidity, hospitalization and mortality. In addition, health care costs and resource utilization for PAH patients and their family caregivers are significant. Vulnerable points in transitions of care for patients include: hospital admission, internal transfers, and discharge to other care settings. This session will focus on the critical issues around transitions of care and an emphasis on the role of the interprofessional team and the need for shared accountability to ensure smooth transitions and reduce hospital readmissions. Objectives: 1) Identify the scope of the problem in PAH including the barriers to effective transitions of care 2) Review considerations in PAH transitions of care 3) Assess medication reconciliation and management for PAH patients 4) Discuss the importance for advanced care planning with the PAH population Presenters: H.  CMSA and the board of directors are pleased to offer CMSA's Standards of Practice for Case Management, 2016 revision, which provides practice guidelines for the case management industry and its diverse stakeholders. The impetus for the 2016’s revision of the Standards is the need to emphasize the professional nature of the practice and role of the case manager.

The 2016 Standards contain information about case management including an updated definition, practice settings, roles and responsibilities, case management process, philosophy and guiding principles, as well as the standards and how they are demonstrated.

Supported by funding from NACDS FOUNDATION, a new eight-hour e-learning program, Comprehensive Motivational Interviewing Training for Health Professionals, is now available. This training uses a sense-making approach (Berger BA and Villaume WA, Motivational Interviewing for Health Care Professionals: A Sensible Approach, APhA Press, Washington, D.C., 2013) to motivational interviewing. The program can be completed within a three-month window and is approved for 8 hours of nurse, pharmacist and physician continuing education credit. It is based on the authors' 25 years of experience in teaching motivational interviewing to health care professionals. It costs $225.

Pat Stricker, RN, MEd Senior Vice President, TCS Healthcare Technologies The Case Management Society of America (CMSA) recently announced the release of its 2016 Extended Conference packages. If you aren’t aware of this program, it is an educational offering that consists of online, on-demand recorded sessions that were presented at this summer’s Annual Conference in Long Beach. It’s a perfect way for case managers who were not able to attend the conference, or for those who attended but were unable to view all the sessions they wanted, to "attend a virtual conference" offering 5, 20, or 35 sessions from the comfort of their own home and obtain continuing education credits (CEs). The packages are offered at a discount for conference attendees and CMSA members. And the courses are available until August 2017, so they can be completed at your own pace. How easy is that? I encourage you to check out the 35 sessions offered from this year’s conference. This made me think about all of the other educational offerings and resource tools that CMSA provides for its members and for organizations who are responsible for providing staff training. It is difficult for case managers to obtain the CEs needed each year for licensure and equally difficult for organizations to find meaningful educational offerings for their case management staff. CMSA helps in this area by providing numerous free educational offerings and resource tools. Other programs are provided for a nominal fee to members and for an additional fee to non-members.

With open enrollment for 2017 only a few weeks away, the Centers for Medicare & Medicaid Services (CMS), announced $63 million in Navigator grant awards to returning and new organizations. These awards will support local in-person assistance to help consumers navigate, shop, and enroll in the wide variety of Marketplace coverage options. "We are committed to making sure consumers have all the resources they need to find the right plan when open enrollment begins on November 1," said Kevin Counihan, CEO of the Health Insurance Marketplace. "We know in-person assistance is critical to connecting individuals and families with quality, affordable health coverage that best meets their needs." Navigators are trained individuals and organizations who help consumers, small businesses, and employees as they look for health coverage options and financial assistance through the Marketplace. Navigators can meet in person with consumers and help them understand the coverage options available to them as well as help them pick, apply for, and enroll in a plan of their choice. Additional funding needed to ensure access to evidence-based treatment On August 31, the U.S. Department of Health and Human Services announced $53 million in funding to 44 States, four tribes and the District of Columbia to improve access to treatment for opioid use disorders, reduce opioid-related deaths, and strengthen drug misuse prevention efforts. In addition, funding will also support improved data collection and analysis around opioid misuse and overdose as well as better tracking of fatal and nonfatal opioid-involved overdoses. "The epidemic of opioid use disorders involving the non-medical use of prescription opioid pain relievers and the use of heroin has had a devastating impact on individuals, families and communities across our nation," said Substance Abuse and Mental Health Services Administration (SAMHSA) Principal Deputy Administrator Kana Enomoto. "These grants will help address the key elements of the opioid crisis by promoting effective prevention efforts, preventing overdose deaths and helping ensure that people with opioid use disorders are able to receive vital treatment and recovery support services."
