 CMSA and the board of directors are pleased to offer CMSA's Standards of Practice for Case Management, 2016 revision, which provides practice guidelines for the case management industry and its diverse stakeholders. The impetus for the 2016’s revision of the Standards is the need to emphasize the professional nature of the practice and role of the case manager.

The 2016 Standards contain information about case management including an updated definition, practice settings, roles and responsibilities, case management process, philosophy and guiding principles, as well as the standards and how they are demonstrated.

Supported by funding from NACDS FOUNDATION, a new eight-hour e-learning program, Comprehensive Motivational Interviewing Training for Health Professionals, is now available. This training uses a sense-making approach (Berger BA and Villaume WA, Motivational Interviewing for Health Care Professionals: A Sensible Approach, APhA Press, Washington, D.C., 2013) to motivational interviewing. The program can be completed within a three-month window and is approved for 8 hours of nurse, pharmacist and physician continuing education credit. It is based on the authors' 25 years of experience in teaching motivational interviewing to health care professionals. It costs $225.

Pat Stricker, RN, MEd Senior Vice President, TCS Healthcare Technologies I recently read an article about a man who took part in an Alzheimer’s genetic early onset clinical trial, and it made me think about how hard it is for patients or their support system to try to find a trial that might be right for them. If they are lucky enough to have a case manager, the process is much easier. However, it is still a long, arduous process for the case manager to sift through hundreds of available clinical trials to find one that meets their patient’s specific needs. A clinical trial, or interventional study, is a research study that uses human volunteers (participants) who receive specific interventions according to a research plan or protocol. The trial may include medical products, such as drugs or devices; procedures; or changes to participants' behavior, such as diet. A trial may compare already available interventions to each other; or compare a new medical product or approach to one that is already available or to a placebo that contains no active ingredients. The goal is to measure certain outcomes to determine the safety and efficacy of the interventions and whether the new product or approach will be helpful, harmful, or no different than the available alternatives.

Patients with pulmonary arterial hypertension (PAH) often undergo multiple transitions throughout the course of their disease, beginning with the journey to achieve an accurate diagnosis and initiate appropriate therapy. The complexity of the disease and therapy requires ongoing interprofessional coordination and system-based approaches to effectively manage care transitions to minimize hospital readmissions and optimize patient outcomes. In partnership with the National Transitions of Care Coalition (NTOCC) and Case Management Society of America (CMSA), a transitions of care white paper has been developed for system leaders and interprofessional clinical team members who treat and manage patients with PAH. The guidance paper, developed by a multidisciplinary advisory board, incorporates essential interventions and pathways designed to ensure effective and safe transitions of care for patients with PAH across healthcare settings.
Eight states in the Delta region are receiving a total of $10 million in grant funding in FY 2016 to reduce chronic diseases that disproportionately affect the region. The funds will support collaborative efforts among health care providers to use an evidence-based model to address diabetes, cardiovascular disease, obesity, stroke and behavioral health. The Delta States Rural Development Network Grant Program requires that local health providers work together across counties and parishes to share resources and expertise, with a focus on strengthening the knowledge of Delta region communities about health risks and disease management. Funded projects are tailored from programs showing evidence of effectiveness in addressing gaps and needs in a community setting and improvements in the health status of individuals. On August 4, the Centers for Medicare & Medicaid Services (CMS) announced the participants in the Frontier Community Health Integration Project (FCHIP) Demonstration, an effort to increase access to care for Medicare beneficiaries in areas of the country where access to health services can be limited because of distance from providers. Ten critical access hospitals (CAHs) in Montana, Nevada, and North Dakota will participate in the Demonstration, which begins this August. The FCHIP Demonstration is another example of how the Administration is working to ensure that Americans receive better care, we spend our health care dollars more wisely, and we have healthier people.
