Yesterday’s Opening Keynote with Josh Luke and Eric Coleman: Leadership and Coordination Beyond the Hospital to the Community

In yesterday’s opening keynote session, presented by Josh Luke, FACHE, and Eric Coleman, MD, MPH, attendees filled the Grand Ballroom to learn about the crucial topic of improving transitions of care. Dr. Coleman spoke eloquently about specific ways in which healthcare providers can work collaborative with community partners to support those in need of care. He emphasized the importance of skill transfer—not simply fixing immediate problems that patients may have but providing them with the skills they need to deal with their conditions. Dr. Luke then took the stage to offer a bracing view of how the fee-for-service payment model has affected patient care, and how new value-based models are likely to affect how healthcare is delivered across the continuum.
