When recommending activity regimens to patients, is one style of dancing an “untapped” resource? A recent study in BMJ Open Diabetes Research and Care examined how tap dance as an activity might affect plantar pressure, postural stability, ankle range of motion and lower extremity functional strength for older patients at risk for diabetic foot complications. This randomized, single-blind, prospective study looked at 40 at-risk patients.

Center of pressure (COP), and its response to activity within the sensorimotor system, is an oft-reported biomechanical measure of balance and gait. COP represents the location on the support surface where the pressure from the soles of the feet would be applied if that pressure were concentrated in a single spot. COP shifts in response to perturbations in balance and activity within the neuromuscular control system making it an important indicator of sensorimotor impairments.

Foot orthoses have been used for decades despite uncertainty surrunding their therapeutic efficacy. Orthoses have been used exclusively to affect neuro-biomechanical input and outcome variables, however, there is emerging evidence that therapeutic efficacy may be affected by a psychological stimulus. Critical appraisal of the literature highlights that there is no holistic model upon which foot orthosis practice is taught, practised nor investigated.

Plantar heel pain (PHP), which affects 4%–7.0% of the community, is associated with impaired health-related quality of life including social isolation, a poor perception of health status and reduced functional capabilities. PHP predominantly affects sedentary middle-aged and older adults, and is estimated to account for 8.0% of all injuries related to running. The condition is characterised by first step pain and pain during weight-bearing tasks, particularly after periods of rest.
