When finishing a case with a resident recently, she noted how smoothly the case went despite having struggled with the same procedure in the past. While the technical aspects of the procedure were no different than the other times she helped perform it, she mentioned that the patient position we chose made all the difference. It was a great observation and brings up a point that I think we seldom discuss in lower extremity surgery: patient positioning.

The bunion deformity, technically termed Hallux Abductovalgus or HAV for short, is much more than just a bump of bone growing on the foot. Technically, it is a 3-dimensional positional misalignment of the entire 1st ray, which includes the great toe and the first metatarsal (Figure 1). Simply put, it’s a biomechanical misfit between the bones that control your big toe — but not just in 1 direction or on one plane.

Diabetes-related foot lesions are a major cause of non-traumatic lower limb amputations and are associated with a high re-amputation rate. Lesions can cause hindrance in activities of daily living, reduce physical function, and lower a patient’s quality of life. Physical therapy is necessary to prevent these limitations. Thus far, there has been limited investigation into the re-amputation rate in patients who have undergone physical therapy.

Optimal postoperative opioid stewardship combines adequate pain medication to control expected discomfort while avoiding abuse and community diversion of unused prescribed opioids. We hypothesized that an opioid buyback program would motivate patients to return unused opioids, and surgeons will use that data to calibrate prescribing. Overall, 934 of 1,880 (49.7%) eligible ambulatory surgery patients were prescribed opioids and invited to participate in the opioid buyback.

Ideal candidate will be qualified from UK, Australia with BSc (Hons) Podiatric Medicine or from Canada. Further education towards MSc along with work experience would be desirable.
