Queen Square Family Health Team (QSFHT) provides accessible, equitable, patient-centered primary care to its
local community. A team of talented interdisciplinary healthcare professionals (IHP) works collaboratively with
Queen Square Doctors, and together, we serve 35,000 patients annually. We offer a wide range of programs and
services including diabetes care, seniors’ wellness, palliative care, mental health, maternal and children’s health,
and gender affirming care. We are a founding member of the Central West Ontario Health Team. We are proud of

We are seeking a dedicated, patient-centered, and highly skilled part time or full time Chiropodist to join our IHP
team. This role involves delivering a full range of exceptional chronic disease related and preventative foot care
to patients of QSFHT in accordance with best practices, and standards of the College of Chiropodists of Ontario.

Key Responsibilities:
• Conduct thorough assessments of patients’ foot disorders or dysfunctions and lower limb conditions
through detailed examinations, medical histories and diagnostic test
• Develop personalized treatment plans tailored to each patient's needs
• Provide expert advice on foot health, footwear selection and proper foot care practices to prevent future
complications
• Educate patients on proper foot hygiene, exercise, and preventive measures to promote overall foot
health
• Prescribing pharmacological therapies within scope of practice (i.e. compounding, oral medications etc.)
• Administer modalities such as Low-Level Laser Therapy for treatment of tendinitis, soft tissue strains,
sprains, injuries, arthritis, wound care to improve wound healing and reduce inflammation etc., and
Infrared Light Therapy (ToeFx) used for treatment of fungal nail infection
• Administer appropriate treatments, such as trimming and shaping nails, removing calluses and corns,
providing basic lower limb wound care as well as performing soft tissue surgical procedures including
injection of prescribed medication (as required)
• Collaborate with fellow chiropodists and / or other members of the IHP team to ensure comprehensive
patient care and health outcomes
• Maintain accurate and up-to-date patient records, documenting assessments, treatments and progress
in accordance with medical standards and regulations
• Ensure all clinical equipment is in satisfactory operating condition and follow safe working practices
• Monitor and manage supplies and inventory necessary for clinic operation in collaboration with fellow
chiropodists and the administrative team
• Uphold high ethical standards and patient confidentiality
Qualifications:
• Completion of a Bachelor of Science and/or Graduate of an accredited school of Podiatry or Chiropody
• Minimum of 2 -3 years of clinical experience; primary care experience is an asset
• Registration with the College of Chiropodists of Ontario is required
• Proof of professional liability insurance
• Knowledge and experience in chronic disease management, diabetes foot care, preventative care,
population, community, and primary care health
Skills:
• Comfortable with electronic documentation and use of technology (QSFHT's EMR is Accuro EHR)
• Flexibility and time management skills

The Chiropodist practices as a member of SRCHC’s multidisciplinary primary health team. The goal is to optimize the mobility, comfort and independence of patients from all age groups and emphasizes health promotion and client education.
This position is expected to work onsite.
Responsibilities:
Direct Patient Care working within the licensed scope of practice:
· Screens new patients and refers for emergency care as needed
· Obtains a complete chiropody history, assessing client's foot health status and developing and implementing a management plan in conjunction with the client
· Assesses, recognizes and treats diseases and disorders of the foot area, including prescribing medications within scope, ordering (through Physicians) diagnostic tests and using palliative and physical treatment modalities
· Provides wound care with appropriate medication prescription and case management
· Performs invasive procedures and minor surgery on the foot as necessary
· Diagnoses, prescribes, fits and manufactures orthotics
· Provides foot care, foot wear and foot health education to patients as needed
· Maintains complete and accurate medical records in accordance with the regulatory agency's standards of practice and the policies of the health centre
· Participates in chart reviews and discussion of problem cases
· Communicates patient test findings and on-going treatment plan to other health care professionals
· Provides home visit services within the CHC catchment areas as required
· Provides foot care services in designated outreach settings
· Makes appropriate referrals to external and internal services to address clients’ social determinants of health
Health Promotion and Education
· Works within own team and other teams across the organization to develop chiropody health promotion activities in the community by identifying chiropody needs and increasing awareness of services

· Collaborates within own team and other teams across the organization to identify and provide health education to target groups
Administration
· Assists in the administration of the CHC by identifying the need for and developing protocols and procedures to improve client services

· Participates in all relevant CHC meetings as required
· Provides statistical information as required by the centre
· Assists in the planning and programming of CHC services by participating in committees and meetings and sharing information obtained through contact with clients, and contributing to planning and budget processes

Qualifications:
· Graduate of an accredited college of podiatric/chiropodial medicine (Designation: D.Ch., D.Pod.M., D.P.M.)
· Member in good standing (current registration) with the College of Chiropodists of Ontario
· Full license, including administering substances by injection and prescribing drugs
· Certification in Local Anaesthesia, Soft Tissue Surgery, Injections, Pharmacology is preferred
· At least 2 years’ experience, preferably in a primary care or community health setting
· Proficiency in the use of computer technology and various software applications
· Strong verbal and written communication skills
· Demonstrated respect for diversity, equity, anti-racism and accessibility with the ability to work collaboratively and positively with others
· Demonstrated ability to work effectively in a multi-disciplinary team environment
· Demonstrated commitment to and knowledge of community-based health care and resources
