For Mother’s Day, I asked my husband to accompany me on a hike. We set out on what seemed like a straightforward 5-mile hike, according to AllTrails™. But after seven days of New England rain, the trail had other plans. What should’ve been a moderate-rated path turned into a mucky, uneven 6-mile journey filled with a few course adjustments, slow climbs, and some unavoidable messes. As we trudged through the mud—sometimes detouring, sometimes stepping straight into the thick of it—I couldn’t help but think about the path we walk as value analysis professionals.

CDC is assisting with an ongoing multistate investigation involving use of non-sterile ultrasound gel for ultrasound-guided percutaneous procedures (procedures that involve skin or tissue puncture). Product testing identified Paraburkholderia fungorum (formerly Burkholderia fungorum) from two non-sterile ultrasound gel products. Genetically similar isolates of P. fungorum were also detected primarily from patient blood cultures; some of these patients were known to have undergone ultrasound-guided percutaneous procedures prior to culture collection.

The COVID-19 pandemic exposed weaknesses in the U.S. health care supply chain, as lockdowns at Chinese manufacturing plants rippled into shortages of personal protective equipment (PPE) and contrast dye used for CT scans, among other medical products. The crisis prompted the formation of a committee by the National Academies of Sciences, Engineering, and Medicine (NASEM), which published a 2022 report, at the request of the U.S. Department of Health and Human Services, titled “Building Resilience into the Nation’s Medical Product Supply Chains.”

The resultant shortage forced hospitals to conserve and reduce their use of IV fluids, which led to canceled surgeries and treatment delays. Such disruptions to hospital inventory have long been hard to predict and difficult for hospitals to navigate.

U.S. President Donald Trump said on Sunday he would sign an executive order to cut prescription prices to the level paid by other high-income countries, an amount he put at 30% to 80% less. In a post on Truth Social, Trump said he would sign the executive order on Monday morning to pursue what is known as "most favored nation" pricing or international reference pricing. The U.S. pays the most in the world for many prescription drugs, often nearly triple that of other developed nations.

Exempting medicines from tariffs is the only way to fulfill his promise to lower Americans' healthcare costs. Many of the medicines that American patients rely upon are manufactured with Active Pharmaceutical Ingredients (APIs) and other components sourced from Europe. And for a variety of logistical, scientific, and regulatory reasons, medical supply chains are extremely complex.
