2017 NCCN Case Manager and Medical Director Program: Transitioning from Patient with Cancer to Cancer Survivor

Nearly 15.5 million cancer survivors were alive in the United States in 2016, and these numbers are predicted to reach more than 20 million by 2026 and more than 26 million by 2040. Unfortunately, many survivors experience physical, psychological, and psychosocial late and long-term effects of cancer and its treatment that can be severe, debilitating, and sometimes permanent or life-threatening.
As the numbers of cancer survivors continue to grow, educating case managers, nurses, primary care physicians, and other members of the multidisciplinary care team on the physical, psychological, and psychosocial late and long-term effects of cancer and its treatment becomes increasingly important. Knowledge of the best ways to guide patients through the critical transition from active treatment to survivorship is of particular importance to the healthcare team. If the transition is optimal, the survivor is more likely to follow surveillance, maintenance therapy, and healthy lifestyle recommendations that can improve their quality of life and decrease their chance of recurrence.
Target Audience
This program is designed to meet the educational needs of case managers, nurses in other roles (i.e., quality management and utilization review), medical directors at managed care organizations, reinsurers, TPAs, and case management and disease management companies who interact with, counsel, and refer patients with cancer to appropriate sources of care and/or make coverage decisions. Programs may also be of interest to nurses, nurse navigators/care coordinators, and physicians who work directly with patients in clinical settings as well as clinical pharmacists in managed care organizations and other settings.
Learning Objectives
Following this activity, participants should be able to:
- Describe common challenges patients face during the transition from active cancer treatment to survivorship.
- Explain the potential role of survivorship care plans in optimizing the transition to the survivorship phase.
- Discuss additional ways to facilitate a smooth transition for survivors after active cancer treatment.

The ACCME/ANCC/ACPE defines “conflict of interest” as when an individual has an opportunity to affect CE content about products or services of a commercial interest with which he/she has a financial relationship.
ACCME, ACPE, and ANCC focuses on financial relationships with commercial interests in the 12-month period preceding the time that the individual is being asked to assume a role controlling content of the CE activity. ACCME, ACPE, and ANCC have not set a minimal dollar amount for relationships to be significant. Inherent in any amount is the incentive to maintain or increase the value of the relationship.
