The National Guard and other military branches are laying off soldiers at all ranks because of downsizing. The Army National Guard is likely to reduce between 15,000 and 35,000. The U.S. Army may lose 80,000 or more. There will be thousands more veterans looking for jobs in addition to those veterans currently looking.

Merger and Acquisition (M&A) activity in health care has reached its highest point since the mid-1990s, according to Irvin Levin Associates. The recent wave of M&A activity does not necessarily indicate that a facility being acquired is having financial difficulty. Many organizations, especially rural hospitals, view a merger, consolidation, or affiliation with a larger system as a proactive way to deal with the uncertainty presented by the Affordable Care Act. A consolidation strategy can provide an organization with access to capital, increased market share, operational synergies in areas such as technology or supply chain, and increased physician alignment.

As a rural hospital CEO struggling to recruit physicians to Ashland, Kan., population 855, Benjamin Anderson believed that "desperation often breeds innovation." When he arrived in 2009, Ashland Health Center's 24-bed hospital had a solo physician assistant who had been "hanging on by his nails" for two years, working with only remote physician supervision and weekend emergency department coverage pieced together with traveling providers. Worst of all, the facility had just two weeks of capital on hand. "We were in peril," Anderson says. "We needed good physicians – not just those who didn't fit anywhere else."

When Carlos Migoya was hired in May 2011 to run Jackson Health System, Miami-Dade County’s safety net hospital system, he had no health care experience. A career banker, Migoya took over a system that bled $428 million over the previous four years. "Our cash supply was in the six-to-eight-days range," says Migoya, who came directly to Jackson after a one-year stint as Miami city manager. He found Jackson lacking budget accountability, expense controls, and unable to collect on certain revenue streams. "Basically, there was no business plan going forward," he says.
