Last week, I saw a patient in my primary-care practice who suffers from H.I.V. and seizures. Lately, the patient—I’ll call him Carl—had been having seizures more often. He told me he didn’t take his seizure medication every day because it reminded him of his illness; he took the medicine only when he felt a seizure coming on, and by then it was too late. Carl had come with a social worker from the housing program where he lived, who told me that Carl’s seizures frightened the staff, since they had no medical training. If the episodes continued, he would have to be transferred elsewhere.
