An FDA safety review has shown that fluoroquinolones are associated with disabling and potentially permanent serious side effects that can involve the tendons, muscles, joints, nerves, and central nervous system, the FDA said. Fluoroquinolone antibiotics should not be used to treat uncomplicated infections the FDA announced. Only patients with these conditions who can't be treated with other agents should receive fluoroquinolones, the agency said.

An recent panel discussion between the CMS Chief Medical Officer (CMO), the AMA CEO, and several major health system CEOs, was interrupted with applause when the CMS CMO asked the panel to identify the biggest barrier to better care delivery and one of the Health system CEOs responded, "I think the biggest barrier is that we're completely worried about our own turf," calling it "a crisis in America, the type of healthcare we deliver, who gets it, the quality, who doesn't get it and the cost. Physicians are worried about what's happening to them, you know. The systems are worried about what’s happening to them, the insurance guys...the pharma people, and it's never really been the year of the patient, right? And if we're going to fix this, it's going to take a huge dose of selflessness, and I don't see this industry talking about that."

There are nightmare stories in healthcare. Lest you assume those cases are exceptions, a new study by patient-safety researchers demonstrates that "medical errors" in hospitals and other health-care facilities are incredibly common and may now be the third-leading cause of death in the United States — claiming 251,000 lives every year, more than respiratory disease, accidents, stroke and Alzheimer’s. The calculation of 251,000 deaths equates to nearly 700 deaths a day — about 9.5 percent of all deaths annually in the United States. Another element of harm that is often overlooked is the number of severe patient injuries resulting from medical error. Some estimates would put this number at 40 times the death rate. It boils down to people dying from the care that they receive rather than the disease for which they are seeking care.

Lawmakers should consider simplifying the rules for health insurance subsidies if they want more people to apply for coverage under the Affordable Care Act (ACA). "On the provider side, we need to consider policies to create alternatives to consolidating physicians around single hospitals. Healthcare reform should be aimed at increasing choice and competition," said one suggestion. We have an inefficient delivery system, spending 30% of the healthcare gross domestic product GDP on wasteful and unnecessary care. "The ACA should replace its convoluted subsidy system with transparent ... tax credits," and encourage the use of health savings accounts.

The USCG panel of 7-9 common physicians has refused to recommend a change in current USCG regulations prohibiting DCs from performing physical exams on merchant mariners. TeamCME President Michael Megehee, DC, sought through a relatively unknown federal regulation to directly petition the U.S. Coast Guard to allow DCs who are certified in the DOT National Registry to perform physical exams on merchant mariners. The Coast Guard has ruled that it will not grant the petition to begin a rulemaking change. There is no current avenue for recourse.

Once upon a time we cured our patients. When we told the Medical doctor such they would get mad. Their view was that we cured no one. All anyone could do was treat conditions. In the philosophy class that I had 98 years ago the instructor of this class, an old man of about 50 or more, came out to teach his class of 75 students sitting in front of him. "THINK!
