In keeping with its overall mission to support case managers, the Case Management Foundation has launched The CMF Certification Scholarship Program. This program will provide several $500 grants annually to case managers who are currently experiencing an economic hardship situation that makes case management certification, prep courses, or exam fees related to certifications impossible at this time.

Applicants must be individuals working toward their case management certification who are currently experiencing an economic hardship .

Applications will be accepted beginning on October 11, 2015, and must be received no later than 12 p.m. EST on November 11, 2015. Scholarship recipients will be announced on December 7, 2015.

Congratulations, case managers! It’s your week. A week to recognize the contributions and commitments you make every day of the year.

According to the History of National Case Management Week, case managers have been honored with their own week since CMSA initiated it in 1999. As other organizations began to establish Case Management Weeks, CMSA approached them to collaborate and have all organizations celebrate during the second week of October in order to strengthen the movement and avoid confusion. In 2000, Tennessee was the first state to issue an official proclamation of National Case Management Week and since then, 43 of the 50 states have also done so. In 2011, the federal government recognized the value of case management when the Administrator of Health Resources and Services Administration at the US Department of Health and Human Services sent a letter to CMSA citing the "vital contribution of case managers in coordinating quality, cost-effective patient services in collaboration with providers and insurers."

Affordable Care Act model designed to improve care for beneficiaries with kidney failure while reducing costs

More than 600,000 Americans have end-stage renal disease (ESRD), also known as kidney failure, and require life sustaining dialysis treatments several times per week. These individuals typically have many health problems, are at higher risk of hospital readmissions, and suffer from fragmented care. In 2012, ESRD beneficiaries comprised 1.1 percent of the Medicare population and accounted for an estimated 5.6 percent of total Medicare spending.  

As part of the Department of Health and Human Services’ approach to building a health care delivery system that results in better care while using taxpayer dollars more wisely, the Centers for Medicare & Medicaid Services (CMS) has announced the participants for the Comprehensive ESRD Care (CEC) Model, a new accountable care organization (ACO) model made possible by the Affordable Care Act and conducted by the CMS Innovation Center. ACOs are groups of physicians and other health care providers who collectively take on responsibility for the quality and cost of care for a population of patients. The CEC Model is designed specifically for beneficiaries with ESRD and builds on experiences from other models and programs with ACOs, including the Pioneer ACO Model and the Medicare Shared Savings Program.

Public comment period offers forum to gather additional feedback and inform future policies

The Centers for Medicare & Medicaid Services (CMS) and Office of the National Coordinator for Health Information Technology (ONC) on October 6 released final rules that simplify requirements and add new flexibilities for providers to make electronic health information available when and where it matters most and for health care providers and consumers to be able to readily, safely, and securely exchange that information. The final rule for 2015 Edition Health IT Certification Criteria (2015 Edition) and final rule with comment period for the Medicare and Medicaid Electronic Health Records (EHRs) Incentive Programs will help continue to move the health care industry away from a paper-based system, where a doctor’s handwriting needed to be interpreted and patient files could be misplaced.

You’re familiar with the term "population health" through posts, articles, and discussions with peers. It seems the term is used everywhere and by almost every segment of the health care marketplace. For more than a decade and a half, the Population Health Alliance has convened organizations in all of these segments to identify and disseminate best practices and advocate for public policy that supports the growth of population health.
 
Now come learn how we all can effectively operate within the framework of population health and strengthen the population health management programs within our respective organizations. On Nov. 2-4, 2015, in Washington, D.C., join the Population Health Alliance (PHA) for its 16th annual Population Health Forum - where population health policy, innovation, and education converge.
 

Supported by funding from NACDS FOUNDATION, a new comMIt eight-hour motivational interviewing e-learning training program for health care professionals will be launched this fall. This training uses the sense-making approach (Berger BA and Villaume WA, Motivational Interviewing for Health Care Professionals: A Sensible Approach, APhA Press, Washington, D.C., 2013) to MI. The program can be completed within a three-month window. It is based on the authors' 25 years of experience in teaching MI to health care professionals. It will cost $225.
