Message from the President
Dear ASHHRA Colleagues,

This month we celebrated our American Independence during what I hope was a very safe and memorable 4th of July. This holiday serves as a reminder of how lucky we are to live in such a great country — a country where our voices can be heard and our actions counted. Collectively, WE make a difference in people’s lives. Our founding fathers fought for this right and the independence to be free. As I watched fireworks on Tuesday evening I thought about what that meant to me and how fortunate I am to be able to exercise that option.

In May, I had the opportunity to visit Washington, D.C., with our Advocacy Committee and conduct visits on Capitol Hill. Of the many privileges we have as Americans, access to our legislators and policy makers is, by far, the greatest privilege. As I made my way to the Senate and House of Representatives, I was astounded by the fact that any citizen can gain access to the U.S. Congress to voice concerns, opinions or suggestions regarding pending legislation for our country and for health care. The walk across the mall from the Senate to the House afforded me wonderful views of our nation’s Capital and the  opportunity to reflect on the momentous actions that have taken place throughout the centuries.

I don’t believe I would have been able to contribute in such a personally significant way if not for my involvement with ASHHRA. Our association has afforded me the opportunities that so many of us can aspire to by getting involved. Even though the 4th of July celebrations are over, and perhaps a food hangover may have resulted from the hot dogs, hamburgers, potato salad and Texas sheet cake, we can’t lose sight of this important holiday’s purpose: our freedom!

Get involved, explore the various ways ASHHRA can provide these experiences for you and take advantage of them. The annual conference is a perfect time to investigate the possibilities. Take the time speak with committee chairs, committee members or board members. You just might find your niche and a way to elevate your professional standing with your employer, in your community and for your country.

A new publication from the National Academy of Medicine summarizes findings from a three-part workshop series on how to improve care for the 5 percent of patients who account for nearly half of the nation’s health care spending.

The 2017 Edelman Trust Barometer global study shows that employees trust their direct mangers more than organizational leaders. Direct managers are viewed as more trustworthy and unbiased, and more likely to listen to workers' concerns. The study is based on the idea of "trust transfer," which it says occurs when people use their trust in familiar situations to weigh their trust in unfamiliar circumstances.

More patients, some of them elderly, are leaving the hospital against medical advice despite the risks. When patients discharge themselves from the hospital against their doctor’s recommendation, most organizations will ask the patients to sign a form that acknowledges that they understand and assume medical and legal risks. However, the situation can get adversarial and is happening more frequently.

Use of telemedicine is exploding in California and other states as insurers increasingly cover tests and treatment overseen from afar. Forty-eight states and Washington, D.C., now provide reimbursement for some form of live video in Medicaid’s fee-for-service model. Still, regulators have approached telehealth with caution.

Health systems are eager to learn about better ways to deliver care. This requires innovation — doing something differently from how it is currently done. In a recent article, Elizabeth McGlynn and Mark McClellan noted that innovations often fail to meet expectations, particularly when they are spread from the initial site that piloted the innovation.

Hospitals that spend more on initial care following patient emergencies see better outcomes than those that spend less at first and rely more on other forms of long-term care, according to a new study from researchers at MIT. Specifically, hospitals that invest more in inpatient care get better results per dollar spent, as opposed to those that assign a higher percentage of patients to skilled nursing facilities when they're discharged.
